Immigration Physicals

At Express Care Urgent Care, ECUC director Dr. Chaudhry is fully Trained Certified Medical Civil Surgeon to perform Medical Exam for USCIS.

A comprehensive medical assessment is the requirement of USCIS, it can take up to two weeks to complete and two plus visits. We handle every aspect of immigration physical including but not limited to

1. A complete Health Exam

2. Phlebotomy / Titers

3. Vaccinations

4. X-ray if needed

5. Referral to outside provider, if needed

Medical Exam for Immigration and forms completion may take two to three visits. At the end you will receive a completed I-693 forms sealed envelope for USCIS and a copy for your record. So if you’re looking for Civil Surgeon to perform USCIS Exam, please call 2032881700 

Immigration physicals are by appointment only, you would need to bring in form I-693 filled with black ink, to make sure the form is most updated. You can print I-693 from the USCIS website. Also, bring

• All immunization, vaccination records, and any other records for this purpose you may have.
• Government, state-issued photo ID, or Passport
• Payment accepted Cash or Credit. NO CHECKS
• The complete process may take one to three weeks, depending on the info you provide and the lab

At Express Care Urgent care we make this process easy for you. It is two Step process. Our Medical Director Dr. Chaudhry is USCIS designated Civil Surgeon. The immigration Physicals exam is conducted by the MD.

In First Appointment you bring

• I-693 filled with black ink.
• Government, state-issued photo ID, or Passport
• Bring any and all vaccine records ( translated into English)
• Required Blood work well done, once we have blood work results, results are negative and you had all required vaccines,( if the results are abnormal, you may require further treatment, which may delay the process.) then we can schedule the second appointment with our USCIS designated MD.

Second Appointment

• The second appointment is about 45 min to 1 hour with MD
• All necessary paperwork will be completed.
• You will have a sealed envelope for USCIS and copy for yourself.
